Zusanli (ST 36) is needled with reinforcing needling technique. Tiaokou (ST 38) located on the affected side is needled with the point of the needle pointing to the direction of Chengshan (BL 57). The patient is asked to move the shoulder joint during the manipulation of the needle. Moxibustion can be used in addition and cupping may be applied for reinforcement of the curative effect.2. Back-Shu acupoints can be moxibusted after needling. The rest of the acupoints can be needled with reducing or mild reinforcing and reducing needling techniques. It is similar to headache and migraine in TCM. The causes are either attack by exogenous pathogenic wind or dysfunction of the liver, spleen and kidney that lead to retention of pathogenic factors in the meridians, or hyperactivity of liver yang, or obstruction by phlegm and stagnation, or failure of qi and blood to nourish the head. [Syndrome differentiaton]1. Wind attacking meridiansFrequent severe headache, onset with the attack of cold and wind, pain involving the neck and back, thin and white tongue fur, taut and tense pulse.2. [Introduction]Intercostal neuralgia is marked by puncturing or lightning pain in the region with the distribution of intercostal nerves. It may be worsened by laughing, sneezing or deep breath. In severe cases, pain may radiate to the shoulder and back. Intercostal neuralgia is usually secondary, primary case is rare. This syndrome pertains to pain in rib-side in TCM. It is usually caused by stagnation of liver qi, damp-heat in the liver and gallbladder, obstruction of blood stasis and insufficiency of blood in the liver and kidney that lead to obstruction or malnutrition of the liver and gallbladder meridians. [Syndrome differentiation]1. Liver depression and qi stagnationDistending pain in the rib-side and hypochondria, oppression and discomfort in the chest, anorexia, bitter taste in the mouth, aggravation with depression and rage, thin and white or yellow tongue fur, taut pulse.2. Obstruction of collaterals by blood stasisFixed stabbing pain in the rib-side, or caused by sprain, contusion and falling with purplish tongue and un-smooth pulse.3.
